Teenagers are required to attend 6 hours of classroom instruction before the written test is administered. Must pass with a 70 or above. Students must also pass the vision and written tests before any DPS paperwork will be given

F) Court Order Name Change
A) Social Security Card*
B) Letter from the Social Security Office stating student's name and SS Number
*For national security reasons, DPS accepts original documents only... NO COPIES.
~~If the NAMES on ID and SS do not match, you will also need a Court Order Name Change.
~~We will return the ID and Social Security Card along with the permit.
3. Verification of Enrollment and Attendance (VOE) Form
Also called the TEA Form, ask for this form from the attendance office at your school.
A parent must sign this form in front of an instructor or have the signature notarized. Be careful to fill in all items appropriately in BLACK INK ONLY.
Documents and Application (1,2,3,& 4) need to be turned in 24 hours prior to the testing date.
30 hours additional driven by the parent
If the student wears glasses or contacts, be sure to bring them on the day of the test. A vision screening will be done as part of the testing.
Teenagers are required to attend class for
hours before testing.
7. When all of the requirements have been satisfied, including the classroom instruction and behind the wheel driving, Keys Driving will issue the TEA Certificate (form DE-964) stating that the student has met all of the requirments.
